Health Builders PC Website Privacy Policy

Last updated: August 16, 2026

This Website Privacy Policy explains how Health Builders PC, doing business publicly as Health Builders, collects, uses, discloses, and protects information through the Health Builders website. It also explains important limits concerning medical information, website forms, cookies, embedded services, and third-party technologies.

Health Builders PC is located at 7540-H Little River Turnpike, Annandale, VA 22003. You may contact Health Builders PC at 703-256-2600 or info@nvhb.org.

1. Application of This Policy

This policy applies to information collected through the public Health Builders website, including information provided through contact or appointment-request forms and information generated through ordinary website operation.

This policy does not replace the Health Builders PC Notice of Privacy Practices. The Notice of Privacy Practices explains how Health Builders PC may use and disclose protected health information in its role as a healthcare provider and describes rights that may apply under the Health Insurance Portability and Accountability Act of 1996 and related law.

Information submitted through a public website can become protected health information when Health Builders PC receives or maintains it in connection with treatment, payment, healthcare operations, or another function covered by applicable health-privacy law. A visitor’s ordinary browsing activity does not automatically become protected health information solely because the visitor viewed health-related content. The legal status of particular information depends on its content, identifiability, context, recipient, and use.

2. Information You Provide

Health Builders PC may collect information that you choose to provide, including:

your name;

your email address;

your telephone number;

whether you identify yourself as a new patient, existing patient, parent, caregiver, or another type of visitor;

your preferred contact method;

the reason you are contacting Health Builders PC;

information entered in a free-text message;

information needed to respond to an appointment request;

communications you send by email or through a website form; and

records of permissions or acknowledgments associated with a form submission.

Free-text fields can contain health-related information if you choose to enter it. Public website forms are intended for contact and appointment requests. They should not be used to submit Social Security numbers, payment-card information, complete medical records, highly sensitive medical details, or urgent medical information.

Submitting a website form does not create a provider-patient relationship, guarantee an appointment, establish treatment, or create an obligation for Health Builders PC to respond within a particular period.

3. Information Collected Through Website Operation

When you use the website, the website, hosting environment, security systems, content-management system, and service providers may process technical information such as:

Internet Protocol address;

browser type and version;

operating system and device information;

referring and exit pages;

pages, links, and files requested;

date, time, duration, and approximate sequence of website activity;

cookie identifiers and consent choices;

server, application, error, and security logs;

information used to detect abuse, spam, malicious traffic, or unauthorized access; and

information generated when embedded content or third-party services load.

Technical information may identify a device, browser, network, or user with varying degrees of precision. Health Builders PC does not describe such information as anonymous unless the applicable data and processing have been verified to meet that description.

4. Forms, WordPress, Elementor, and Website Services

The website is built with WordPress and Elementor and may use WPForms or Elementor forms for contact and appointment requests. These systems may process form entries, form metadata, timestamps, Internet Protocol addresses, consent records, and delivery information according to the active configuration.

The exact form-storage location, email-delivery path, retention period, administrative access, encryption settings, spam-prevention method, hosting arrangement, and business associate agreements must be governed by Health Builders PC’s approved technical and privacy configuration. This policy does not state that WordPress, Elementor, WPForms, email, hosting, or any other system is HIPAA compliant merely because the system is used by a healthcare practice.

Where information submitted through a form is protected health information, Health Builders PC will handle that information under its Notice of Privacy Practices and applicable law. Website visitors should use the form only for the limited purpose stated beside the form.

5. How Health Builders PC May Use Information

Health Builders PC may use information collected through the website to:

respond to contact and appointment requests;

communicate about a requested appointment, question, or service;

determine which member of the Health Builders PC team should review a request;

operate, maintain, secure, and troubleshoot the website;

prevent spam, fraud, abuse, malicious activity, and unauthorized access;

preserve records of consent, requests, and communications;

improve website organization, performance, accessibility, and usefulness;

comply with legal, regulatory, licensing, billing, recordkeeping, and professional obligations;

establish, exercise, or defend legal claims;

investigate incidents and enforce website terms; and

carry out other purposes disclosed at the time information is collected or authorized by you.

Health Builders PC will use protected health information according to its Notice of Privacy Practices and applicable law.

6. How Information May Be Disclosed

Health Builders PC may disclose website information when reasonably necessary to:

service providers that host, maintain, secure, transmit, or support the website and communications;

contractors and professional advisers performing authorized services for Health Builders PC;

healthcare team members who need the information to respond to a request or carry out an authorized healthcare function;

payment, billing, record, or administrative service providers when applicable to the request;

government authorities, courts, regulators, law-enforcement agencies, or other parties when required or permitted by law;

protect patients, visitors, Health Builders PC, service providers, or others from fraud, abuse, security threats, or harm;

support a merger, reorganization, financing, sale, transfer, or other business transaction, subject to applicable health-privacy and confidentiality requirements; or

fulfill a purpose you request or authorize.

A service provider that receives protected health information on behalf of Health Builders PC must be handled under applicable health-privacy requirements, including a business associate agreement when required. A website privacy policy, cookie notice, or general consent banner does not serve as a HIPAA authorization for a disclosure that otherwise requires a valid authorization.

Terms such as sale and sharing can have technical definitions under different privacy laws. Health Builders PC will evaluate any activity covered by those definitions, provide legally required choices, and update this policy before implementing a material practice that changes this disclosure.

7. Cookies and Similar Technologies

The website may use cookies, local storage, pixels, tags, scripts, and similar technologies for essential operation, security, preference storage, form functionality, performance, analytics, or embedded content. Some technologies may be set by Health Builders PC. Others may be set by a service provider after the applicable feature loads.

The website uses a consent-management function intended to provide choices for categories of nonessential technologies. Essential technologies may operate because they are required for security, network management, form delivery, user-requested functionality, or other core functions.

More information appears in the Cookie and Tracking Technologies Disclosure. Cookie choices do not authorize a disclosure of protected health information that requires a separate legal basis.

8. Google Maps and Other Embedded Content

The website may display Google Maps or other embedded third-party content. Loading an embed can cause the third party to receive technical information such as an Internet Protocol address, device information, referrer information, cookie identifiers, and interaction data. Third-party privacy terms govern the third party’s independent processing.

The website may use consent controls for nonessential embedded content. You may obtain the clinic address and telephone number without loading an embedded map.

9. Google Services and Analytics

The website may use Google services enabled through Site Kit by Google. A Google service can be installed, connected, or active independently of another Google service.

If analytics or another measurement service is enabled, it may process technical website-use information described in Section 3 according to the service configuration and available consent choices. Health-related form contents are handled under Section 4 and, when applicable, the Health Builders PC Notice of Privacy Practices and applicable law.

10. Security

Health Builders PC uses administrative, technical, and physical safeguards selected for the nature of the information and the systems involved. Website security can include encrypted transmission, software updates, access controls, logging, backups, malware protection, spam controls, and vendor safeguards, depending on current configuration.

No website, email system, network, or storage system can guarantee perfect security. You should avoid sending highly sensitive information through ordinary website forms or ordinary email. Contact Health Builders PC by telephone if you need instructions for a more suitable communication method.

11. Data Retention

Health Builders PC retains website information for periods reasonably necessary to carry out the purpose for which it was collected, comply with legal and professional obligations, preserve security and consent records, resolve disputes, and enforce agreements. Different categories of information can have different retention periods.

Protected health information and medical records are retained under applicable federal and Virginia law and Health Builders PC’s record-retention policies. Website logs, form records, email records, backups, security events, and cookie choices can have separate retention periods. Health Builders PC will update this policy when a precise public retention period is required or materially useful.

12. Your Choices

Depending on the technology and applicable law, you may be able to:

use the website’s cookie-preference control;

block or delete cookies through your browser;

avoid loading optional maps or other embedded content;

contact Health Builders PC by telephone instead of submitting a public form;

request an accessible format or communication accommodation;

unsubscribe from commercial email if Health Builders PC sends commercial email; and

exercise rights concerning protected health information under the Notice of Privacy Practices.

Blocking cookies can affect website functionality. Browser privacy signals and opt-out preference signals will be handled as required by applicable law.

13. Children and Parents

Health Builders PC provides care for children, but the public website is directed to adults, parents, and caregivers. Children under 13 should not independently submit personal or medical information through the website. A parent, legal guardian, or authorized caregiver should contact Health Builders PC on the child’s behalf.

The Children’s Online Privacy Protection Act applies to websites and online services directed to children under 13 and to operators with actual knowledge that they collect personal information online from a child under 13. Health Builders PC does not intend the general website forms to invite independent submissions from children under 13. If Health Builders PC learns that a child submitted information without appropriate involvement, it will evaluate the information and take legally appropriate action.

Healthcare privacy rights involving minors can depend on the type of care, the minor’s legal authority to consent, parental rights, and federal and Virginia law. Those issues are governed by the Notice of Privacy Practices and applicable law.

14. Virginia Privacy Law

The Virginia Consumer Data Protection Act applies only when statutory thresholds and other conditions are met and contains an entity-level exemption for a covered entity or business associate governed by HIPAA. Other Virginia laws can apply to medical records, data security, breach notification, consumer protection, and the use of a person’s name or likeness.

This policy does not claim that a particular statutory exemption resolves every privacy issue. Health Builders PC will evaluate obligations according to the information, activity, entity status, and law in effect at the relevant time.

15. External Links

The website may link to medical organizations, government resources, manufacturers, social-media services, review platforms, or other third parties. Health Builders PC does not control the privacy, accessibility, availability, security, or accuracy of third-party sites. A link does not establish endorsement of every statement, product, service, or practice of the third party.

16. Changes to This Policy

Health Builders PC may update this policy to reflect changes in law, technology, website functions, vendors, or information practices. The revised policy will display a new “Last updated” date. A material change will be communicated in an additional manner when required by law.
